Waiver & Liability Release
Participation in Hay Of The Heart Offerings
In booking a session with Hay of the Heart, I am voluntarily participating in Reiki and holistic treatments, workshops, retreats, activities and services provided. I confirm I am over the age of 18 and understand that services provided are intended to enhance relaxation and increase communication within my body.
I understand that by participating, I need to provide the following personal information about myself that includes sensitive information about my health, and situation and that this information is strictly confidential in nature and will not be shared with any third party without my written consent, unless required by law;
client’s name, address, mobile number, email address
client’s emergency contact name and phone number
client’s doctor’s name, address and phone number
any relevant medical conditions, surgical procedures and medication
I understand that only the practitioner will have access to information in my file in order to provide the service.
I understand that Reiki uses a combination of hands-on and hands-off placements on various parts of my body, always avoiding sensitive and private areas, and that I will remain fully clothed.
I understand that it is my responsibility to consult a doctor prior to and regarding my participation in these services, that these services are not to be used in place of medical assessment or treatment by a doctor, and I understand that the Reiki practitioner is not a doctor, psychiatrist, nor medically trained.
I state that I am physically fit and have no medical condition that would prevent me from comfortably lying down on a massage table for the duration of the service provided.
By participating, I knowingly, voluntarily, and expressly waive any claim I may have against my practitioner for any risks, injuries or damages sustained during these activities, including any advice given by the practitioner.
Long Distant Services (Reiki sent to me in my home without the practitioner present)
In addition to the points made above, the following applies to long distant Reiki services:
I acknowledge that long distant treatments are to take place during an agreed time with my practitioner, in a space where I am able to fully relax and give my full attention, without distraction, ideally while lying down in a quiet space in my home and I accept full responsibility for my home space and the health and safety precautions within this space.
Cancellation & Reschedule Policy
I understand that unforeseen circumstances may arise, leading to the need for cancellations or changes to your session plans. If you are unable to make your booking, please notify me as soon as possible via email at hayoftheheart@gmail.com
If you need to reschedule or cancel with more than 24 hours notice, I can offer you either;
a full refund
transfer your booking to a future date, depending on availability
If you cancel with less than 24 hours notice or fail to show;
No transfer option will be provided
No refund will be given
If Hay Of The Heart has to cancel due to sickness or low numbers (in the case of a group activity), you will be notified via email and a full refund and/or transfer of ticket of the same value to a future appointment or event will be offered, dependent on availability.
Please do not attend if you feel unwell or display any Covid-19 symptoms, to ensure the safety of your practitioner and other clients.
Parking
Please note that vehicles are left at the owner's risk and I do not accept any responsibility for any damage caused to your vehicle while attending an appointment.
Agreement
I understand that by providing this informed consent and agreeing to the terms and conditions outlined above, I am assuming full responsibility for the service(s) provided and I hold harmless the practitioner, any advice given and the location where the services are provided.
